Registration should take place in advance of the conference wherever possible. A registration form can be downloaded here and should be completed and sent together with the registration fee to Millbrook Medical Conferences Ltd.
Alternatively, registration and payment can be completed securely online here.
PLEASE NOTE THAT THE REGISTRATION FEE WILL INCREASE AFTER FRIDAY 24th SEPTEMBER 2010 AND AGAIN ON THE REGISTRATION DAY.

The registration fee for Europe AF includes:
- Admission to all Scientific Sessions
- Admission to the Exhibition

Payment must be in Sterling by cheque, bank draft or transfer, VISA or Mastercard and sent together with your completed registration form to Millbrook Medical Conferences Ltd.

| Cancellation of Registration |
The registration fee paid will be refunded less 25% (to cover the cost of administration) should delegates cancel their registration prior to 29th October 2010. No refund will be made for cancellations after this date. If requested, conference material will be sent to delegates who have paid the registration fee.
CPD Accreditation has been applied for. Please ensure you sign the attendance register.
