REGISTRATION FOR EUROPE AF

Registration should take place in advance of the conference wherever possible. Any registration forms should be completed and sent together with the registration fee to Millbrook Medical Conferences Ltd.
Alternatively, register online with our secure online registration and payment facilities.

PLEASE NOTE THAT THE REGISTRATION FEE WILL INCREASE AFTER FRIDAY 25
APRIL 2008 AND AGAIN FOR ‘ON THE DAY’ REGISTRATIONS.

The registration fees are detailed on the registration page. The registration fee for Europe AF include:

• Admission to all Scientific Sessions
• Admission to the Exhibition
• Use of the Internet Café
• Refreshments and Lunch on each day
• All materials relating to the Conference
• Ticket to the Gala Dinner on Sunday 15 June

PAYMENT

Offline payment must be in Sterling by cheque, bank draft or transfer, VISA or Mastercard and posted together with your completed registration form to Millbrook Medical Conferences Ltd.

CANCELLATION OF REGISTRATION

The registration fee paid will be refunded less 25% (to cover the cost of administration) should
delegates cancel their registration prior to Friday 30 May 2008. No refund will be made for
cancellations after this date. If requested, conference material will be sent to delegates who
have paid the registration fee.

CPD ACCREDITATION

The Royal College of Physicians have awarded Europe AF 12 credits for full attendance.
